Use this form when making a donation by cheque and/or if you wish to make a Gift Aid declaration (e.g. if you've already made a donation without a declaration).

Your cheque, of course, has to be sent in the post.  You can either print and send this form in the post or you can submit it to us over the Internet. 

If you are a sponsor making a donation towards a volunteer's fundraising target, please say so in the comment field and give the volunteer's name.  If you are a volunteer making a donation to an AidCamp you are coming on, please put in the name of the AidCamp.
